My 111th birth handled.

My patient Rebecca, 17 y.o, G1P0 (means first pregnancy) came to the clinic on 4/09/00. I did an IE (internal examination) and she was 7cm open, 90% effaced, +1 station and intact Bag of Water. She was very quiet and handled her labor well.

Then after a while she wanted to push, so I did another IE to make sure she’s 10 cm so she can start pushing but she was already 8-9 cm. I lie down on the bed next to hers. I was about 5am in the morning she told me that she really wanted to push. She was almost 10cm. There was still an anterior cervical lip above the baby’s head. I let her breath and lie on her side. There should be no cervix above the baby’s head because it will be swollen with the pressure and it makes it harder for the baby’s head to come down.

After awhile she can’t helped but push so I pushed the anterior cervical lip up above the head of the baby so it will not come out together with the baby’s head. She was really a good pusher. After few times pushing her Bag of Water ruptured with slight meconium stain (meaning her baby already pooped inside which means the baby is already distressed). She was pushing for a while and there was only a little progress. So I let her on full squat then on birth stool.

Then the baby’s head was already visible and then after few pushes the head crowned with a caput formation. Then the head was out and was rotating 180 degrees. That was great! Usually, the baby’s head only rotates 90 degrees. The baby was out at 6:45am to a healthy baby girl named Jezelyn, with apgar score of 9/9, weighed 2523 grms/ 5.9 lbs. Glory to God.

The caput was huge found near in the frontal lobe. It means the baby’s was not properly position during the birth process. We immediately gave the baby a vitamin K to prevent bleeding. Usually we gave vitamin K after few hours after birth but in cases of huge caput we need to give it immediately. The baby was doing great.
The placenta was out at 7:00am but the membranes were not complete so we put the patient on methergine tabs. The patient has a labial laceration but no need of suturing. She only lost 100cc of estimated blood loss. I endorsed her to my co worker Sara Jones. Present at birth: Julie Tinio, Kristen Grubb, Lynn Jones.

2nd stage- 46 mins
3rd stage- 6 mins
Admitted- 6 hrs.

PROTECT AGAINST TETANUS

As nurse or midwife we need to be sure that all pregnant women who are coming in for prenatal check-up should be injected with TT (tetanus toxoid)injection.

Every pregnant women living in an area where there is neonatal tetanus should be vaccinated against tetanus at the first antenatal visit or every pregnancy. If she has not previously had the full course of tetanus protection, she should be vaccinated again 1-2 months later, then again 6 months after that.

CARE DURING PREGNANCY

If pregnant women get even the most simple, basic and cheap antenatal care it can make their pregnancy and birth much safer. What is done during an antenatal or prenatal visit depends of the level of training on the health worker, the equipment and laboratory tests which are available to them, how often the come to visits and the health problems which are common in the area.
Antenatal care aim to :
• Develop kind and respectful relationship between the each woman and the health workers, so that she will attend for antenatal care and be more likely to talk about any problems. She many also be more accepting of any advice given.
• Find and treat any illness the woman already has.
• Identify which women can safely deliver at home or in the clinic and which women are more likely to have complications and should deliver in the hospital.
• Explain to women the danger signs that can occur during pregnancy, labour and delivery so that they seek help early.

EDUCATED WOMEN MAKE HEALTHIER CHOICES.

In my experience I observe that pregnant women are highly educated are more concern and interested on what is going on in her pregnancy. They tend to make good decisions. Education gives people skill and confidence, and makes them aware of the choices they can make in their lives. Educated women are more likely to marry later, have their first pregnancy later and have fewer children. In addition, they are more likely to know about contraception, start to use it earlier and rely on modern rather than traditional methods. Educated women are more likely to attend for antenatal care during pregnancy.

Getting a good start

The conditions of a woman’s life affect her health and that of her children. For women to be healthy during pregnancy and childbirth they need: good nutrition, especially during childhood, several years of education (beyond primary level), respect and consideration in their home and their society.
By the time a woman becomes pregnant, many things will have happened in her life which affect her and health. For example, the food she ate as a child – the type and the amount – will make a big difference.

Crazy nurse/midwife with oxygen on

A day to remember with me and my best friend. We had only one patient this time. The patient was still in early labor so we told her to walk around while we were doing crazy things unimaginable.
We didn't know what went to our heads. It was just out of blue we decided to have oxygen on and of course, we had to take pictures.
The funny thing is we really do look like real patients. Aren't we? We could be good models for oxygen, don't you think? We do accept though that when we are together we get so crazy and wild. lol.

Happy bday to my youngest brother

My cousin gave birth

My cousin gave birth to a very healthy and cute little baby girl named Kate Nichole. She gave birth in the hospital. Everything was find with no complications. Kate is her second baby. I delivered her first baby at the birth center. Another angel is added to our family. Everyone is very happy welcoming our new member of the family. Both my cousin and her newborn are doing well. Thanks God.

My cousin is due this month

My cousin is due this month. When I did the prenatal exam, I palpated that the baby's head is already engaged (meaning the head moved down to the pelvis). It means that in two or three weeks she will have her baby. I explained and showed her pictures of baby.

By nine months, most of the baby’s bones have hardened, although the skull is still pliable. She is shedding most of the downy covering of hair as well as the creamy vernix caseosa that covered her whole body. She spend the next few weeks just putting on weight.
Soon your baby settles down lower in your abdomen in preparation of birth and may seem less active. Skin is pink and smooth and the child measures 19 to 20 inches in length. Lungs are mature and at approximately 8 pounds the baby is ready for life outside your womb. Soon you’ll see her face to face!

A baby named after me

One of the joyous things being a nurse-midwife can have is when the parents name their baby after caregiver's name. I felt so privileged and honored when I learned that my patient decided to name her baby after me. I did asked them many times though if they are really sure of it. They were 100% sure. The baby is very cute like me lol... She is super cute but she had a hip dislocation and was referred to the doctor for it.

My first twin delivery

During this holiday seasons, I am reminded of the twin babies that I delivered 7 years ago. I wonder how they are doing. I hope they grew up well and in good health. It was a surprise twins. Actually, the day before the patient was in labor I did her prenatal exams and found out that her fundic height is much larger than her Age of Gestation. I referred her for an ultrasound to confirm my suspicion. Due to lack of financial resources, she did not have one. So when she came in labor, I thought that she only has one big baby inside her. She denied to have history of twins in her or her husband's family. I was so surprised when the baby came out very small. I suddenly looked at the tummy, it was still very big. So I did an IE and there was another baby. The second baby came out after 20 minutes. We did resuscitated them both but they recovered very well. To God be the glory. Everything went well.

Synchronized-pushing on supported squatting

This was one of the days when we had no patients. We do crazy things. Sometimes we pretend to be patients or patients' husbands. These are my colleagues doing synchronized- pushing on supported squatting. I really love them. They are so great!
If patients see us what we do when we have no patients, they will think we are all crazy. It is fun fun fun!!!

Prenancy is a turning point

For me, pregnancy is one of the most amazing,miraculous and wonderful things God created. It is also the most wonderful experience a woman can have.

Pregnancy is one of the turning points in a woman's life, as well as her partner's. during pregnancy a woman's body undergoes a lot of changes. She needs to understand what these changes are, and what they mean.
a pregnant woman needs the full support of her husband or partner after all, it's not just the woman's concern. It is a family concern.

Another person who plays a crucial role during the whole process of pregnancy is the midwife or the doctor. With their help, they can help make the pregnancy safer- and even joyful.

Noodles treat after catching babies!!

After a long night shift, nurses and midwives got hungry after delivering babies. We usually treat ourselves with noodles! It is so much fun eating noodles early in the morning when we have time to eat. There are many nights that we can't even sit for a minute or two because we are busy catching the miracles of life.

Seldom sleeping chances

When we got a chance to sleep we sleep when there are no laboring patients. It is just very seldom we got this chance though. Mostly we are awake during our night shift. Sometimes I am wondering why babies loves to come out to this world during night time. If we could just schedule them when to come out, life would have been easier. It is okay though because the moment baby comes out, all of the doctor's, nurses' and midwives' tiredness and muscles pains will be gone. Babies are so amazing, it seems like they bring magic to the world. They melt hearts.

Eight month- fetal development

Overall growth is rapid this month and the baby grows as much as a half pound per week. At eight months of pregnancy, the baby may weigh 4 to 6 pounds and is gaining about half a pound per week. Layers of fat are pilling under her skin. Soon she will turn head down in preparation for her coming birth.
Most body organs are now developed with the exception of the lungs and if the baby kicks you’ll not only feel it and also be able to see it on the outside. Tremendous brain growth occurs at this time and the fingernails begin to extend past the fingertips.

Seventh month- fetal development

The seventh month of pregnancy is the beginning of the 3rd trimester of the pregnancy. By month’s end, the baby is 14 to 16 inches long and can weigh 2.5 to 3.5 pounds. The organs continue to mature.
If you expose your belly to a bright light you may get a quick reaction. The fetal baby can now inhale, exhale and eve cry. Eyes have completely formed. And the tongue has developed taste buds.
His body fat is beginning to form in preparation for life outside the womb.
If born at this time, she would be considered premature baby and requires special care.
